Our interdisciplinary team works together to create a care plan for your loved one that addresses their physical, spiritual and emotional needs to ultimately make sure they are comfortable. We have years of experience and can address pain and any other symptoms with medications, therapies and many non pharmacologic measures. Comfort is of paramount importance and we rely on your guidance and feedback to make sure your loved one’s needs are being met.

Hospice care is for those who have been determined, by a physician, to have a prognosis of 6 months or less if the terminal condition naturally progresses. Palliative care is provided to patients with chronic or life-limiting conditions who continue to seek aggressive/curative treatments but need symptom management support to improve their quality of life.

We provide 24/7 care and support to our patients and their loved ones. There is always an on-call registered nurse available to take your questions or listen to your concerns. If the need arises, we will be at your side to walk you through any healthcare crisis your loved one may be experiencing.

We do our best to make sure through the admission process that hospice care is the right fit for you or your loved one both from a medical stand point, while following Medicare guidelines, and with what we have to offer as a company. Given that, you always have the right to revoke hospice care at any time to seek curative care. A small percentage of patients in hospice care nationwide are discharged from hospice care for having an extended prognosis if there is no sign of decline while in hospice care.

We are currently accredited by Community Health Accreditation Partner, CHAP. This is the highest level of accreditation.

Discussing hospice care is a difficult conversation to have. The best time to begin learning about what hospice care has to offer is now. Being informed empowers both you and your support system so that when hospice care does become necessary it is far less overwhelming.
