What is Home Health Care?

Ultimate Caring began in 2009 primarily as a home health care company. Home health care is short term medical care provided to patients recovering from surgery, a recent illness or injury. Patients who also need help managing a new diagnosis or have worsening symptoms from a chronic disease would qualify them for services. Care is provided to patients in the comfort of their residence and is led by their primary physician. Our interdisciplinary team members work with patients and their families to create goals that will help them get back to their daily activities.

We communicate regularly with your doctor and update them on your progress. Our goal is to help our patients feel better, regain their independence and meet their care plan goals as determined by their doctor and our interdisciplinary team. Home health services are more convenient and far less expensive that frequent or extended hospital stays. Once we receive a referral from a physician, our first priority to is schedule an appointment to assess the patient’s current health status and determine if home health care services are appropriate.
